There are two ways to access on our comprehensive benefits plan INDIVIDUAL and GROUP

INDIVIDUAL

Through Associate membership in CSJB, Associate Members can obtain access to health benefits offered by the CSJB Health and Welfare Trust Fund.
WE ARE NO LONGER ACCEPTING INDIVIDUAL APPLICATIONS.                                  

Important plan details for all members: 

  • Copies of Marriage Licenses and Birth Certificates are required for dependents' coverage
  • Member must be actively working full time. 
  • In-network only coverage except for emergency. See SBC for details.
  •  There is no access to BCBS online portal. You will be sent your Explanation of Benefits
    (EOB) in the mail directly to your address.
  • The plan is one-year minimum contract.

  • All individual enrollments need to be completed by the 15th of the month at 5pm CST.  If the 15th falls on the weekend, enrollments must be completed by the following Monday by 5pm CST.  (Completed enrollments include payment information, marriage licenses and birth certificates)

GROUPS OF 5+

Groups that have W2 employees
Through membership in CSJB, members can obtain access to health benefits offered by the CSJB Health and Welfare Trust Fund. 
Groups of 5+ that already have an established payroll, can join PEO 4 ME and access our benefits package for their employees.

  • Groups with 5+ full-time employees will have access to our benefits package of Medical, Dental, Vision and Life stated in the chart below
  • Employees can waive coverage but a minimum of 50% participation in the plan is needed to qualify for group coverage.  A minimum of 5+ lives enrolled for all groups
  • Waived employees still pay the $32 for dues and receive 10k of life insurance.
  • Waived employees must have other coverage and submit proof. 
  • All groups will be quoted separately for Dental, Vision & Life and Accident according to the census that is provided
